I give permission to my student to participate in this retreat as well as other activities with Calvary Youth Ministry. 

I understand that there will be adult supervision at the event. This document is valid until revoked in writing by the undersigned and delivered to the church office at 14201 N. 32^nd Street, Phoenix.

I give my consent for the youth group counselors, chaperones, retreat staff and/or qualified medical personnel to act on my behalf in securing and administering necessary emergency medical care and treatment for him/her.

I hereby authorize Calvary North Youth Ministry to act according to their best judgment in any emergency requiring medical attention, and I agree to indemnify and hold harmless their officials and employees affiliated with the program from any, against any, and all liability for any injury which may be suffered by my child out of/or in any way connected with the participation in the Youth Ministries. Calvary North is hereby released from responsibility to pay for such service rendered. 

In all meetings, retreats, or other events with Calvary North Youth Ministry, I am responsible for my actions. I understand that the following guidelines will be followed:

1. The use of drugs, alcohol beverages, and tobacco are prohibited.

2. All conduct shall be in keeping with the highest respect for all persons and always stay together.

3. All individuals will be expected to participate in group activities.

4. All youth will assist in leaving areas clean and undamaged.

5. All individuals will travel together, unless specified differently.

6. All individuals will abide by rules set by the Youth Ministry Team concerning the possession and use of

cell phones and other portable devices

I, the undersigned, have read and understand the Covenant of Conduct above.
